Complete application form & documents
3 copies of passport valid 6 months from return date (pages with passport number, visa stamps and contact info
2 copies of second semester validated Ateneo ID
Recent passport size picture
AISIS printout showing yearly and cumulative QPI
Clearance from the college Guidance Office and from the Office of the Student Affairs
2 copies Medical Clearance from the Loyola Schools Infirmary (fit to travel – 1 copy original, 1 photocopy)
2 signed copies of the Declaration of Fitness
2 signed copies of the Conditional Agreement
PAYMENT OF FEES
Reservation Fee Deadline (Euro 400): TBA
Reservation fee is NON-refundable but DEDUCTIBLE from the final payment.

NOTE
Surcharge for late payment: EURO 100
Study Tour Package INCLUDES: round trip airfare; tuition and materials abroad; board and lodging with 3 meals a day; local transportation; cultural activities and entrance fees.
Study Tour Package DOES NOT INCLUDE: tuition in Ateneo (6 units); Comprehensive travel insurance; medical tests, visa fees, and travel agency fee.
